Dear Parents,
As you may be aware we have provided sports physicals for your athletes each year in the fall. We will be doing those physicals this year on July 29th at our American Fork Office located at 226 North 1100 East in American Fork beginning at 6:00pm. The charge is $30.00 for each athlete and the majority of the money collected will be returned to your school for your sports program. This is a great opportunity for your athletes to get their physicals and also earn money for the sports program at your individual schools.
